The Community Fellows Program (CFP) aims to ground emerging nonprofit and community leaders in the systemic context of social change work to develop key management and leadership skills.

The role of the Care Manager is to deliver the 6 core services (comprehensive care management, care coordination and health promotion, comprehensive transitional care, individual and family support, referral to community and social support services, and use of HIT link services) in a person-centered manner in order to meet the needs of the individual, the OPWDD valued outcomes, the objectives of the People First Transformation, and the state requirements.

The Health Care Specialist is primarily responsible for providing emergency medical treatment, limited primary care and health protection and evacuation from a point of injury or illness. Health Care Specialists are often called "combat medics" in the Army, because some Soldiers in this MOS are assigned to deploy with Army combat units, and provide emergency medical treatment directly on the battlefield.Other Heath Care Specialists are assigned to military hospitals and clinics to assist doctors and nurses with the health care needs of patients.

-Ensure that the six health home core services are implemented to meet the person centered needs of the individuals receiving services. Including but not limited to the developing, writing, monitoring, and implementation of the Life Plan.-Supporting the individuals receiving services to facilitate Life Plan meetings.-Provides advocacy for people receiving services to assist them to reach their full potential.-Ensure benefits are maintained (Medicaid).-Record and maintain records in the Care Management Platform.-Attends department/team meetings, trainings, supervisions, etc. as scheduled and in accordance with agency practice and policy.-Completes all required trainings.-Travel throughout the designated service area to meet with people receiving services and their interdisciplinary team.-Maintains confidentiality.

A habilitation specialist helps mentally disabled clients learn social skills and, when possible, skills for living independently.Responsibilities include:Serves as a liaison between Agency, residences, and families. Serves as advocate and provider for all issues pertaining to individuals receiving services.Develops programs and Day Habilitation plans for persons served.Implements active treatment plans for program participants, both inside and outside of the building.Supervises non-professional staff in the implementation of Day Habilitation plans.Participates in individual program planning process.Orients, trains, evaluates, delegates, supervises the work of and recommends the hiring and disciplining of subordinate staff.Recognizes and observes important relationships with Service Coordinators for participants’ program development and referral for other kinds of assistance as needed.Maintains attendance and other required records/paperwork.Utilizes community resources, as appropriate for implementation of Day Habilitation plans and community integration.Carries out basic administrative tasks related to caseload and other selected assignments as directed by the Program Director or Program Coordinator.Plans group, individual activities and assigns staff to implement.Trains and instructs support staff in daily routine, program plan and general procedures and practices of the group.Attends regularly scheduled supervisory staff meetings.Prepares and adapts materials and plans and sets up furniture and equipment for program activities.Participates in activities outside of activity room when necessary, i.e. trips, pool, gym. Enters swimming pool with program participants as required.Selects and utilizes educational materials.Teaches persons served to become more independent.Initiates precautions within the established framework to assure that own work is completed in a manner free from recognized hazards.
