Worked in HIV Homeless Outreach and Mobile Engagement (HHOME) program to serve highest need population with HIV and homelessness. Performed intensive case management linking case load to medical care, housing, substance abuse treatment, mental health support, ADL/IADL accommodation, benefits guidance, legal assistance, and HIV care. Practiced whole persona and harm reduction philosophies and inter-personal skills in supportive counseling, goal planning, and motivational interviewing.Developed expertise in rapport building, complex hospital discharging, and working with high-utilizer populations.

Transitioned team of case managers and community health workers from Q/I program into novel program in medical case management. Medical case management program layered medical supportive services to shelter resources and provided medical respite placement for high acuity patients.Built out policy, and procedures, role descriptions, service list, and case assessments for building program governance. Marketed program to community stakeholders and built collaboration between inter-county case management teams. Also worked with local hospitals and respite facilities to better navigate barriers and alleviate challenges in discharging populations dealing with homelessness.

Coordinated services and managed program for COVID-19 Quarantine and Isolation for Salt Lake County's unsheltered populations. Collaborated with shelters, hospitals, and rehabilitation centers to ensure safe transitions from isolation recovery. Oversaw daily operations in security, data collection, hotel hospitality, food/supplies stocking, and meal services. Communicated weekly Q/I trend data for city, county, and state reports. Drafted policy and procedures, training workflows, and safety compliance. Following FSC's discontinuation of motel-voucher program, assisted shelters in adopting their own quarantine-in-place programming. Continued as point-person for navigating difficult Q/I related community problems and crises.

Worked in small team to co-run Salt Lake City's Covid-19 Q/I Hotel for unhoused. Managed discharge between hospitals, resource centers, and streets for over 300 patients and coordinated reentry into congregate settings. Major responsibilities included providing meals, hotel hospitality, medical check-ins, contract tracing, and room turnover/decontamination. Other responsibilities include medical assessment/policy drafting, medical problem solving, and mediating various crises in property damage, patient grievances, domestic altercations, and arthropod infestations..

Engaged in shelter and street outreach in COVID-19 vaccine enrollment, education, and record verification. Delivered PCR and NAAT testing in full PPE. Analyzed infection trends, compiled COVID-19 data, and documented encounters in EPIC HIS.

Performed case work advocating and providing rent relief for housing and economically vulnerable individuals. Mediated landlord-tenant disputes, housing contracts, and goal planning for financial resilience. COVID-19 grant compliance required role to meticulously scrutinize rental contract and verifying employment and tax documentation. Also assisted with Community Services Block Grant writing and data collection.
