As a Discharge Planner at Signature, I play a critical role in ensuring the smooth and effective transition of patients from inpatient care back into the community. My primary responsibilities include assessing patients’ needs, collaborating with the healthcare team to develop comprehensive discharge plans, and coordinating post-hospitalization care and services.I work closely with patients, their families, and a multidisciplinary team of healthcare professionals to identify the most appropriate resources and support systems. This includes arranging follow-up appointments, securing outpatient treatment, coordinating with social services, and addressing any barriers to a successful discharge. My goal is to promote continuity of care, reduce readmission rates, and support patients in achieving optimal mental health and well-being.
