Over a 2 ½ year period I was responsible for overseeing a number of programs including the Commonwealth Home Support Program (CHSP), Home and Community Care (HACC) and the COVID Response Team.My responsibilities include:• design and deliver on program and strategic priorities,• strategic input into the design and delivery of strategies and actions relating to optimising program performance across the organization• advising Operations Leaders on other program optimisation opportunities including but not limited to; financial and reporting improvements, practice excellence, service and system improvements and growth opportunities.• advice and input into business development and tender opportunitiesMy role also included a key role in Care Connect’s COVID Advisory Team and has included the development, implementation and oversight of the COVID Response Team.

Over a 3 year period I was responsible for the oversight of Care Connect’s transition out of disability funded services. My key role was to:• provide leadership to a national client services team, • develop and manages business plans, • manage senior stakeholder relationships.• ensures their team meets operational and reporting KPIs and growth targets.My role oversaw the transition of 2000+ state funded Disability clients to NDIS.

Over a 3 year period I was responsible for the management oversight of the Practice and Quality Team. The Practice and Quality Team provided an integrated approach to system development and a commitment to best practice across the entire business. The Practice and Quality Team assisted in meeting the organisation’s strategic goals, and provided support to:• develop and implement projects/ new programs• embed service model• identify and implement practice change to meet both internal and external drivers• support client services to deliver consistent best practice nationally• implement a quality management system based on a continuous improvement model• ensure our systems and processes result in high quality services for our clients• support business development teams in preparing tender submissions• maintain required accreditation.Successfully developed and implemented a number of projects including:• National Practice Audits• Review of key practice tools• Suicide Response Protocol• Outcomes Measures Project• Practice and Quality Training • Program Implementation Plans

My core function as Client Services Manager was to provide leadership to the local client services team. This included:• identifying and develop ongoing business opportunities• pursuing growth opportunities, • People management and development• meeting all operational and reporting obligations and growth targets Project Lead for the development and implementation of multiple programs including:• Mental Health Community Support Service,• Aboriginal Access and Support Program, • Individual Support Package (ISP) Planning and FacilitationMember of executive group for FSP demonstration project. Provided expertise and knowledge to support project development and implementation.

Over a 2-year period I was responsible for managing Care Connects ABI Programs. My core function as an Assistant Client Services Manager was to provide leadership to the local client services team. This included:• People Management and Development• Practice and Quality Management• Stakeholder Engagement• Assist in Business Development• Meeting all operational and reporting obligations and growth targets

Employed as a Care Manager in both the Slow to Recover and Transitional Care Program. My role included assessment and planning to support clients to return to community-based living.

Member of the Victorian Coalition of ABI Service Providers (VCASP) Inc., a peak body for ABI service providers in Victoria. Supported the board to implement key strategic plans to:o promote the needs of people with ABI, and their carers, through consideration of needs on a statewide basis. o promote planning, development, evaluation, research and education in the ABI field. o promote the development of improved services and facilities for people with ABI and their carers. o initiate discussion and provide advice to Governments, and other funding and policy bodies, regarding issues affecting persons with ABI and development of services to meet identified needs. o establish linkages with peak disability bodies to ensure that the needs of people with ABI and their carers are adequately represented on, and presented to, these bodies.

Employed within both the Community Access Program and the Statewide ABI Case Management Program as a specialist Acquired Brain Injury Case Manager. My role included assessment and planning to support people with acquired brain injuries to identify supports and services to support skill development and to maximise quality of life and community participation.My role included 2 years working on the Slow to Recover program providing case management within a community based slow stream rehabilitation program for people with catastrophic brain injury.

Employed as a specialist Acquired Brain Injury Case Manager undertaking assessment and planning to assist people with acquired brain injuries to identify supports and services to support skill development and to maximise quality of life and community participation.

Employed as a Community Access Worker working with people with dual disabilities to provide community-based community access opportunities. My role included developing and implementing programs and outings based on client’s choice and preferences.
