Self Employed Geriatric Care Management: Home visits to elders with family members often present. 1st visit was an Intro. to working with a Care Manager. Next visit until completed, Comprehensive Geriatric Assessment to learn about their problems/issues across a wide range of areas. Physical health, Mental Health, financial status and management, Memory assessment, interaction with family members, social services needed etc. After completion of the CGA, I did the interventions that were needed, which included referrals to home care agencies, mental health providers, obtain financial benefits that were possible for the client, Determine in cooperation with the client's doctor, if Conservatorship was needed. and much more. Whenever client wanted to terminate my services or vice versa, either the client or I, would give at least a week's notice.
