Dr. Susan Mani is an executive physician leader uniquely poised with the strategic and operational abilities to transform healthcare. With years of practical, hands-on expertise, Dr. Mani has extensive experience redesigning care delivery throughout the full continuum of both health systems as well as health plans dedicated to significant improvements in population health outcomes.Currently, Dr. Mani serves as the AVP for Population Health for Humana. Dr. Mani is responsible for the design and implementation of clinical programs to fulfill Humana's vision to go beyond a traditional health plan and become a clinical company. She leads enterprise teams, using human centered design principles and agile methodologies, in creating connected, member-centric experiences for Medicare Advantage populations focused on improving their health outcomes. Dr. Mani has advanced numerous capabilities necessary to advance Humana's clinical transformation.Prior to her current role, Dr. Mani was the Chief Population Health Officer, Vice President of Clinical Transformation, and principal thought leader for population health program development and operations for LifeBridge Health, a $2.5 billion health system serving over 1 million patients annually. She provided strategic vision and direction to improve health outcomes and effectively manage cost and utilization of over 200,000 lives in value-based care contracts and the CMS total cost of care model. She oversaw care management, data analytics, the clinically integrated network, chronic disease management programs, and digital platforms to support all population health activities of a fully integrated health system. She demonstrated a proven track record for breaking down complex situations with poor outcomes and reconstructing them into high performance scenarios. Today’s healthcare landscape is complex, rapidly changing, with an increasing need for high performance in value-based care.
