Review and Abstract relevant clinical data from EMR to select appropriate CPT codes and ICD 10 code, sequencing diagnosis per patient medical record for surgical cases from all sections and most appropriate Primary diagnosis (All Chronic conditions, status) from the Operative Report and other Medical records as per the guidelines.• Assigning the appropriate ICD 10 PCS codes for Inpatient accounts and Surgery accounts as per the client requirements• Assigning appropriate Modifiers HCPCS codes for Drugs and supplies• Assigning the appropriate Revenue codes as per the Client updates.• Assigning appropriate DRG , Comorbidities and and Major Comorbidities and POA status as per the guidelines.

• Accurately assign the ICD 10 code, sequencing diagnosis per patient medical record for physician billing in ENM and Surgery specialty• Review and Abstract relevant clinical data from EMR to select the appropriate CPT .• Responsible for performing claims review based on medical records documentation and compliance guidelines to ensure accurate patient billing and coding.• Accurately assign the Modifier and HCPCS when required.• Ensure all the coded data accurately reflects service provided base on the documentation• Responsible for Reviewing EOB’s for various insurance carriers and completing necessary action for denial received• Responsible for raising Info request if required information missing and completing necessary action on receiving the response against the request
