Responsibilities: Identify barriers to create family specific treatment goals. Provide crisis support to families 24/7, assessed safety, and created safety plans. Teach families life skills, parental skills, employment skills, and problem solving skills. Prepared detailed confidential documentation for state and agency records, and provided testimony for the court. Building trusting relationships with families, foster families, attorneys, Department of Human Service ongoing caseworkers and child protective workers, schools, and outside community agencies.Provided resource referrals; Medicaid services, food assistance, therapy services to families and assisted them in gathering required documentation. Supervised family interactions between parents and children, and provided transportation assistance. Completed CPR, First Aid, and certifications requirements. Accomplishments: Completed services with an above site average of treatment goals met of 85%. Recognized by three different Department of Human Service employees for the high level of service provided to families, high level of preparation to help a family out of crisis, and the ability of address the family’s issues and provide a quality report for court. Responsibilities: Organizing the schedules of four counselors, school psychologist, and social worker. Involved in all aspects of enrollment, registration, and scheduling for students. Entrusted with personal and confidential information regarding families and students including IEP, 504, health plan, homebound services, court order paperwork, and Power School. Assisted families in setting up Parent PowerSchool account by identifying the barriers, teaching, and walking them through the steps. Developed a professional relationship with several outside agencies including: GHAEA, PIMIC, Lutheran Family Services, Catholic Charities, Choices, Military, Heartland Family Services, Clarinda Academy, Alpha School and other school districts. Coordinating the Teammates program at Thomas Jefferson High School by scheduling visits, weekly newsletter, and identifying and problem solving with the matches.Accomplishments: Develop a successful Teammates program in the building evidence by more than doubling the number of contact hours between matches, and increasing the number of matches.
