• Working in an adults inpatient physical rehabilitation unit. Effectively managing a diverse caseload consisting of individuals with various physical and mental health conditions and neurological diagnoses. Organising new case referrals, prioritizing cases, and ensuring timely completion of assessments. Conducting comprehensive moving and handling assessments and seating assessments in both hospital and home settings. Management and completion of standardised and non-standardised assessments and report writing Assessment of Activities of Daily Living including washing, dressing and kitchen tasks within the hospital setting and home environment. Establishing and nurturing relationships with healthcare professionals, social care providers, partner agencies, and multidisciplinary teams. Cultivating rapport with service users/patients to better understand their needs and provide appropriate support. Collaborating with and training junior colleagues to enhance teamwork and professional growth. Manage and maintain service user data together with review their information within company electronic computer systems Continued Professional Development, keeping abreast of current evidence-based practices in line with HCPC and RCOT standards of practice.

· Managing a complex caseload with a variety of physical and mental health conditions and neurological diagnoses in people's homes.· Caseload Management - Managing new case referrals and prioritising cases and completion of assessments· Completion of moving and handling assessments and written moving and handling plans· Management and completion of time regulated assessments and report writing•Completion of adapted rehousing reports on behalf of the Housing team.•Recommendations and review of minor and major adaptations within people's homes.· Making recommendations for daily living equipment within people's homes. · Build new and maintain relationships with health and social care professionals and partner agencies and multi disciplinary team.· Develop relationships with service users/patients· Working together and training junior colleagues.Manage and maintain service user data together with review their information within company electronic computer systems· Continued Professional Development, keeping abreast of legal practices and developments in better ways of working and supporting communities.

·Assist the Occupational Therapist with interviewing clients admitted to hospital· Carry out assessments on stroke and elderly medicine patients.· Assess functional state of patients· Complete neurological assessments, standardised cognitive assessments and Barthel Index within the ward as and when required.· Assessment for the provision for equipment· Document clinical notes· Liaise with multi-disciplinary team members as appropriate.

· Deliver personal care for clients· Assist with dietary and fluid needs· Documenting notes regarding patient care· Moving and handling of patients· Reporting clients condition to multi-disciplinary team as needed.· Assist learning disability clients with assisting personal care, leisure needs and cooking
