As a Cardiac Clinical Case Manager at Asante, I educate and support patients with various cardiac conditions, helping them find a healthy, balanced lifestyle in the cardiac medical world. I build meaningful relationships with my patients, as I empower them to manage their own health and wellness. I also collaborate with other healthcare professionals to ensure optimal care coordination and patient outcomes.I have been a cardiac nurse at Asante's Heart Center since May 2021, where I gained valuable experience in the step down intermediate care unit, taking care of patients fresh out of lung surgery and valve replacement procedures. I also worked with STEMI and post open heart surgery patients, in a fast paced environment and with large patient to nurse ratios. I have skills in patient care management, primary care nursing, and advanced cardiac life support. I have a BSN in Nursing from Oregon Health & Science University, and a BS in Outdoor Education from Southern Oregon University. I am passionate about human connection as I strive to provide the best care possible for my patients.
