• Works in close collaboration and communication with physicians and other members of the health care team in order to minimize duplication of effort, promote teamwork, and ensure seamless coordination of services within and between teams and across campuses.• Assists with patient evaluation and treatment to further their understanding of significant social and emotional factors underlying a patient’s health problem(s). Provides clerical support to assist in the delivery of quality health care services.• Serves as a concierge - greets and communicates with patients or visitors, including collecting payments and co-payments.• Performs clerical functions, including, but not limited to registering/checking in patients for appointments, answers telephone/switchboard; schedules walk-in patient appointments, collecting payments, producing end-of-day reports. Document patient phone call/in person encounter in EHR for call routing/resolution.• Performs patient registration and check-in confirms patient demographic and insurance information; assists patients with kiosk registration. Performs patient check-out: schedules follow-up appointments or confirms future appointments.• Maintains patient medical records according to standards; uploads Right Fax documents into EPIC. Responsible for basic EMR/HER scanning.• Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.

• Performs receptionist/secretarial duties and maintains confidential records, supplies and equipment per hospital and unit specific standards.

• Assists clients and families across the lifespan with access to healthcare and health related social services, through client action planning and care coordination with clients and providers. • Navigation services provided by this position include screening and assessment of health and social needs and barriers to comprehensive healthcare, education, referrals to the social services and healthcare systems, and linkage to community services encompassing a wide variety of health and social needs domains. • Works as a member of the interdisciplinary Health Net client services team to provide services to individuals/families by addressing social correlates of health to achieve health equity for individuals/families.
