 Conduct client interviews individually, with family or in a group setting. Assess and identify current situation, capabilities and other factors to determine needed services.  Develop a service plan. Meet with clients to review and solicit input. Conduct follow up to monitor and assess the effectiveness of services provided. Document case history and progress towards service plan. Maintain comprehensive client flies. Prepare reports as requested by Supervisor.  Assist clients in identifying and obtaining available benefits as well as social and community resources. Serves as liaison between family services, medical/psychological health professionals and other contacts to help individuals and families. Meet regularly with Director of Clinical Invention to staff case load. Provide on-going program evaluation and recommendations to the Director of Clinical Intervention for continuous growth and quality.

Compile information regarding the clients’ pre-disaster circumstances. Develop a recovery plan. Meet with clients to review and solicit input. Conduct follow up to monitor and assess the effectiveness of services provided. Document case history and progress towards recovery plan. Maintain comprehensive client flies. Prepare reports as requested by Case Management Supervisor or Program Manager.  Assist clients in identifying and obtaining available benefits as well as social and community resources. Serves as liaison between family services, medical/psychological health professionals and other contacts to help individuals and families in post-disaster events. Met regularly with Case Management Supervisor to staff case load. Provide on-going program evaluation and recommendations to the Case Management Supervisor and Program Manager for continuous growth and quality.
