Integral member of the stroke team who champions high quality stroke care for patients and families across the continuum of care. Effective team member who collaborates with internal and external partners to promote evidence-based patient-centred care. Utilized coaching, mentoring and facilitation skills to support teambuilding and change to enhance stroke care. Effectively implement changes as needed to ensure goals are achieved. Applied analytical skills to interpret raw data for quality performance indicators analysis and monitoring for the Stroke Services Metrics committee. Verification of stroke quality metrics supported the committee in developing action plans to improve stroke performance. Led members of the committee in identifying process changes that could benefit stroke care delivery because of my ability to review data and translate that into clinical practice. Successfully led clinical teams to deliver high quality stroke care and achieve Stroke Distinction at recent survey.
