Clinical Pharmacist
Current· Manages all admitted, observation, and boarding patients, evaluate all orders for appropriateness, and completes full medication reconciliation at time of admission and discharge· Perform medication discharge counseling and device demonstration at bedside prior to patient discharge· Responds to Medical Emergency Codes and traumas in the ED (ACLS, PALS, and BLS certified)· Provide aid in antibiotic selection and stewardship, manage vancomycin and warfarin dosing in admitted patients, and other drug dosing or related questions as requested· Serves as USP 797 Officer to ensure compliance with 797 and all aspects of sterile compounding, including training, biannual testing, and monitoring· Rotates through outpatient pharmacy and family medicine clinics· Serves as Anticoagulation Clinic Provider where we see patients in clinic and fully manage their warfarin regimen under a collaborative practice agreement
