• Evaluates and admits patients to hospice services.• Conducts initial meetings with patients and families to inform them about hospice services and obtain consent for care.• Performs follow with facilities, families and care teams• Sustains and increases referral sources with hospitals, nursing homes, and other community sources. • Documents visits and phone calls with patients, families and referral sources• Conducts discharge planning of hospital patients that are being transferred home to be admitted onto care with Hospice.• Mentors and trains new registered nurses.• Communicates with doctor’s for initial plan of care.

Waterford Estates is a 250 rooms facility.Assisted Living DirectorDirectly responsible for the oversight of Assisted Living floor licensed for 60 beds to ensure that emotional, physical, psychological and safety needs are met through all programs, service and activities.• Provides leadership and direction for a team of 32 direct reports. Consisting of 30 certified nursing assistants and 6 LPN’s. Including but not limited to scheduling, on-going training and education, disciplinary action and evaluations.• Develops and monitors documentation systems to ensure that all regulatory timeframes for review of assessment, service plans and resident needs are met.• Collaborates with residents, families and outside care giver to ensure that are at appropriate level of care.• Manages supplies and costs associate with the Assisted Living budget.• Ensure compliance with all state regulatory requirements including mandatory reporting to the state.• Assesses potential residents to determine and recommend level of care at Waterford Estates, including field assessments in homes or skilled medical units.• Adheres to all policies and procedures as indicated by the Joint Commission.• Strives to maintain a safe working environment through the prevention of accidents, preservation of equipment and the achievement of safe working practices.• Opened the nursing department in April of 2014 in the memory care unit. Hired, staffed, and trained each nurse in the new unit. Read and followed each regulation as assigned by joint commission for the new unit.
