I manage a partial hospitalization program that serves adults with chronic mental illness and substance abuse.

•Successfully worked with psychiatrists and nurses within the hospital to develop multidisciplinary treatment plans for patients with diagnoses such as schizophrenia, major depression, opioid/alcohol dependence, personality disorders and patients experiencing suicidal and homicidal ideation •Rapidly assessed patients on a biological, psychological and social level to determine the most appropriate treatment and aftercare plan to meet the patient’s needs•Recognized by supervisor for strength in advocating for patients’ rights to team members, insurance agencies and aftercare providers in order to provide affective, patient-focused care•Diligently worked with patients to organize successful discharge plans that included after care, shelter and psychological education around their diagnosis, treatment and plan to remain safe to themselves and others•Persistently advocated for patients to organizations such as the Department of Mental Health to initiate adequate support and care to allow the patient access to services that could reduce the affects of mental illness and improve quality of life•Supported patients as a dual diagnosis social worker through the process of their detoxification protocol while implementing a discharge plan that would allow them to continue their substance abuse treatment at a lower level of care

•Worked on a clinical team under the Child Behavioral Health Initiative delivering in- home, client focused services on the South Shore of Massachusetts•Provided on site crisis intervention to families, working with local crisis teams and police departments to deescalate and stabilize while promoting safety and control•Communicated with collaterals including schools, Department of Children and Families, and Department of Youth Services to create a wrap around process•Supported youth through transitional phases such as aging out of the Department of Children and Families’ care and creating an independent lifestyle•Successfully worked with the National Human Trafficking Resource Center to provide a client with a safe outlet from a sex trafficking ring and later provided resources towards recovery•Actively participated in individual supervision to discuss challenging cases, gain insight and work through transference and countertransference •Diligently completed 5-6 daily progress notes, quarterly treatment plans and crisis, safety plans for each client in accordance with the Massachusetts Behavioral Health Partnership

•Maintained a case load of four to five residents, ages eleven to twenty-one-years old to address issues of trauma, gang activity, juvenile delinquency, substance abuse and family discord•Co-facilitated daily groups that focused on topics such as: substance abuse and prevention, relationship building and coping skills using a Dialectical Behavioral Therapy model•Actively offered a clinical perspective to a resident’s behaviors while on the unit at Behavioral Management Team meetings to determine their privileges for the week•Upheld resident files that included treatment, safety and service plans on the Juvenile Justice Enterprise Management System in accordance to the Department of Youth Services standards•Acclimated new residents to the unit by presenting Prison Rape Elimination Act (PREA) material, delivered the GAINS test to evaluate risk to self and others •Attentively collaborated with staff to maintain an orderly, safe and secure locked unit for all persons
