I am responsible for carrying out an individual special needs plan of care, collaborating between participants, family, members, caregivers, and care coordinators, updating goals, and determining approaches and strategies to help the participant achieve the highest quality of life. Timely completion of 40 various competency-based courses every year. My participants receive services in their residence and out in the community to provide supportive opportunities, using inclusion and coping strategies through different situations.
