Duties: • Assisting individuals with a wide range of developmental disabilities and their families to navigate the NYS OPWDD system to obtain services that meet their needs/goals. • Regularly meeting face to face with individuals/families to discuss goals, needs, and provide service related documentation. • Coordinate bi-annual “Life Plan” reviews with an individual’s “Circle of Support” to update and create a new, comprehensive plan based on the individual’s needs, wants, goals, and currently utilized services. • Refer individuals/families to services that meet their needs. • Attend CSE, IEP, and other meetings important to the individual/family to provide support/advocacy. • Work with service providers to tailor services to the needs and goals of the individual/family.
