• Provide cross-site specialized and intensive psychosocial support to identified caseload of Community Care program members with severe and persistent mental health and substance abuse issues; initiate specialized behavioral and harm reduction care plans for identified caseload and as requested• In conjunction with the Behavioral Health Supervisor and Administrator serve as a resource to teams in problem-solving member behavioral health issues and in care planning for effective management of behavioral health issues, including Behavior Support Plans and Behavior Intervention Plans• Serve as a behavioral health consultant to Community Care social workers; identify appropriate specialized service resources and interventions for best management of behavioral health issues and related family/caregiver and legal concerns • Meet regularly with the Behavioral Health Supervisor/ Administrator to review cases; assist the Behavioral Health Administrator to develop and refine policies and procedures; monitor utilization of psych and related specialized services• In conjunction with the Behavioral Health Supervisor and Administrator identify educational needs of social work staff and interdisciplinary teams then plan and implement trainings• Lead support groups and treatment groups at designated sites within the scope of certification or licensure• Provide psychotherapy to designated program members under the supervision of a qualified person as required by the Department of Health and Family Services for Licensed Clinical Social Workers

-Provided individual psychotherapy to Washington County residents to address mental health conditions as well as Substance Use Disorders-Provided mental health treatment and monitoring of mental health commitment and settlement orders in conjunction with the Washington County Mental Health Treatment Team-Engaged in cross-program, inter-department treatment consultation and mental health service delivery
