Over 16 years of Geriatric long term care. Early experience working in Canadian long term care nursing homes since graduating in 2002. Proficient in admissions, discharges, psychosocial assessments, facilitating care conferences with residents and families and actively working on care plans. Work well with management team leaders and front line staff members to enhance residents individual care needs. Assisting residents with Medical Assistance applications, including CADI/Elderly waiver services. Assisting residents and working with Relocations services returning home or Assisting Living placement. Collaborating with Hennepin County liaisons and medical insurance care coordinators with individuals health plan benefits. Experience in education: Resident Bill of Rights, PHIA/ HIPPA standards, advocating for residents care needs, Reporting/ advocating residents rights: MAARC/ Vulnerable Adults. Assist individuals with Guardianship/Conservatorship designation. Providing support in a compassionate and caring environment to our growing elderly population with strong advocacy on maintaining residents rights and individual care needs.
