Conducted monthly interactions with patients to follow up with their current medical conditions and medications. Discussed new conditions they are experiencing, educated and informed physician then followed back with patient for any new instructions.

Assisted with obtaining appropriate patient information from hospitals. Submit to insurance companies for approved skilled stay. Then keep communication open with the rehabilitation facility for patient progress then update the insurance company for additional days needed. Assist the skilled center for appropriate discharge and any DME that may be required for home care.
