• Manage multiple projects simultaneously: Case Investigation and Contact tracing interviews, Covid 19 vaccine scheduling, skilled nursing facility employee and patient outreach • Work in conjunction with Santa Clara County Public Health Department and Heluna Health to deliver daily contact to the public in order to provide assistance and obtain confidential information • Interpret and explain public health policies and procedures to community members • Interview as many as 10 cases and contacts per day and conduct up to 50 vaccine outreach calls per day • Exercise cultural competency to work with interpreters and non-English speaking individuals • Elicit close contacts of cases and promptly notify them of exposure and proper quarantine procedures • Educate and inform individuals how to isolate or quarantine, understand their risks, and stop the chain of transmission • Facilitate community and clinical resources referrals as needed for qualifying patients • Use of computer applications to enter data, medical history, flight information, work and school exposures/outbreaks • Document interviews in Salesforce: CalConnect database • Achieve Santa Clara County’s goal of having one the highest vaccination rate’s in the country; over 80% of eligible residents have completed their vaccination.
