I also delight in being a member of the London Hospices LGBT network & a published author of articles and multiple book chapter contributor. have worked for the County for 12 years now and always a part of Adult Social Care. I started my career as a Duty Social Worker and have enjoyed the challenge of working to the post of Operational Service Manager in both the North and West localities. I have recently divided my working schedule between operational service management in West and that of Social Care Lead for the National End of Life Care Programme. Whilst with the programme I recently co-wrote The Social Care framework - supporting people to live and die well. This very much highlights social care and its role in End of Life care planning and support in partnership with health and universal services. I am now really excited that I will be using this expertise in the following months as i undertake a project in service development for End of Life care in Essex. The intention is to look at what Essex has currently and is doing well and how we can improve our services for the future so that the people of Essex can indeed live and die well. This work will include providing our front line workforce with additional tools and skills with which they can with confidence have the important discussions with individuals and their carers as to how they would like their care to be provided and where their preferred place of care will be. Additionally working across health and social care provision and commissioning to identify a clear care pathway for all.
