I am a highly experienced, Professional Pharmacist and Clinical Manager, with more than fourteen years’ expertise acquired working in senior team leadership, policy development, and public facing roles: always doing everything I can to ensure the best possible clinical outcomes to the highest professional standards. I am highly motivated, passionate about what I do, and have excellent communication and collaborative skills. Amongst my other professional qualifications, I have an M. Pharm Degree from Kings College London (2004), am a Certified Independent Prescriber (University of Hertfordshire), and have completed a Pathway Course in Primary Care Pharmacy Education, as well as a PIT Stop Advanced Diabetes Course. At the moment I am working towards qualification in advanced practitioner roles, and I am Registered with UK General Pharmaceutical Council.I have a Postgraduate Certification in Clinical Pharmacy Practice (2020), and a particular interest in the management of patients with complex diabetic conditions.In my current role as a Clinical Pharmacist at Norbury Health Centre in London (since 2018), I am supporting successful level 3 diabetic care strategies by initiating and maintaining Glps and insulin programs. In addition, I play a key part in ensuring the Practice either achieves or exceeds QOF targets; oversee management and staff training as part of EPS 4 roll out within the Practice, and worked with a team of local volunteers, pharmacists, and social workers during the COVID pandemic to process repeat prescriptions on behalf of especially vulnerable patients.I have also completed numerous audit assignments: including a review of patient use of a short acting bronchodilator (with no Inhaled corticosteroids prescribed), and (currently) a review of diabetic patients with a focus on minimising cardiovascular renal risk, and I oversee prescribing practices, as well as ensuring prescribers follow local formulary requirements.On those occasions when primary care prescribing is unavailable, I refer patients directly to secondary care centres.Earlier in my career, from 2016 to 2018, I was Pharmacist Manager at the Haria Pharmacy in London: managing and supervising a skilled team of pharmacy technicians, and dispensers, as well as mentoring and supervising a pre-registration pharmacist.I am proud too of having spearheaded, a number of new NHS and Private Clinical Services, including a travel clinic, flu clinic, and various PGD services.To find out more, send me an InMail, or contact me at seemathapliyal05@gmail
