I’m a credentialed Wound Care pharmacist and my specialty is in Wound Care and Compression therapy and provision of education and services for acute and chronic wound care treatment, including skin tears, post-operative wounds, burns, leg ulcers and oedema. I am keen to develop referral pathways and allied healthcare networks to optimise access to wound care and compression therapy in the community.Home Medicine Review provides medication management for patients with multiple comorbidities, recent changes in medications, discharge from hospital, experience adverse reactions and so on. My role is to review the administration and effectiveness of patient's medication therapy and provide information to patient's GP about any potential medication related issues and provide recommendations to reduce polypharmacy, optimise quality of life and treatment outcomes in line with the patients' beliefs and values.
