As a boom operator, I refueled aircraft while they were still flying by manually operating a set of flight controls to essentially fly the 'boom' into the refueling receptacle of aircraft that were flying less than 50 feet behind our aircraft. Then our aircraft would transfer fuel down the boom and into the other aircraft so they could fly longer.
