Case Manager

Evaluated patient data that was existing at the time the protocol was submitted to the IRB for initial approval.

Screened referrals for appropriateness and perform admission to facilitate the transition of the patient and family into the home health program. Also responsible for Case Manage patients and families to ensure symptom management, patient and family education, medication management, coordination of home health services, and providing excellent communication with the patient’s physician.
