About Timothy Hart

With over thirty years of experience in the health care industry, I am an expert in creating and implementing value-based contracts (VBCs) and alternate payment models (APMs) in the physician and home care space. I have a proven track record of securing and managing VBCs/APMs with national and regional payers, resulting in substantial savings for the payers and increased revenue for the providers. I have also developed and implemented a physician-led population health program, including palliative care, that aligns with the goals and outcomes of the VBCs/APMs.As the Vice President of Payor Relations at Team Select Home Care, I oversee all managed care and commercial payers in 11 states, with an annual revenue of $400 million.
