In collaboration with the Houston Police Department (HPD) and The Harris Center Crisis Call Diversion (CCD) a specialized unit. Conducts crisis assessments to reduce or manage symptoms of serious mental illness or prevent admission of an individual to a more restrictive environment. Care Coordinator is paired with another clinician (LPHA/MLC) and deployed into the community to provide crisis services and intervene with at-risk individuals using intervention strategies to lower risk and avoid unnecessary law enforcement involvement.
