The Change Management Consultant is responsible for supporting Specialized Community Service Programs (SCSP's) for key target populations. As a Change Management Consultant I work with operational and program leaders to develop and implement innovative models of care that result in improved processes and workflows in Island Health as well as across the continuum of care including with non-Island Health service providers (e.g. local physicians, non-profit agencies and private practitioners). Responsible for facilitating the adaptation of practices to support change management and alignment of services with Island Health Primary Care Networks (PCN's). Change Management Consultants, works with others (including patients/family, different community agencies, Divisions of Family Practice, Practice Support Team and a range of Corporate supports) to facilitate and monitor behavioral change necessary to establish data collection and evaluation of SCSP's in the communities they work. Furthermore, I foster the development of multi-stakeholder, multi-sectoral groups that identify and take action on priority issues in their communities.

Reporting to the Program Manager, I support and facilitate MHSU staff throughout the Central and North Vancouver Island Health Service Areas to attain excellence in clinical practice through the development, provision and evaluation of clinical strategies, education and clinical resources for ongoing improvement of services for individuals experiencing problematic substance use and concurrent disorders (mental illness and substance abuse/dependence.) I Apply a broad knowledge base of the health care environment, the science and art of addiction treatment, educational theory and models, program/curriculum development and evaluation, organizational, teams and systems development, planned change, process consultation and research utilization to provide role modeling, leadership and consultation and to promote a positive learning culture and advance professional practice.Responsibilities:• Facilitate educational experiences. Curriculums include, Suicide Risk Management (SRM), Motivational Interviewing (MI) , Core Addictions Practice (CAP), Trauma Informed Practice (TIP), Applied Suicide Intervention Skills Training (ASIST). • Design educational strategies to support and change practice.• Evaluate current educational programs and integrate current methods and tools.Key Accomplishments:• Co-wrote updated Core Competency Profile for Island Health Mental Health & Substance Use.• Completed requirements to achieve Registered Trainer status with Living Works’ ASIST.• Reviewed and Re-designed CAP curriculum to better serve MHSU needs across the life span.

Reporting to the Clinical Coordinator and working as a member of a multidisciplinary health care team, responsible for providing intake, assessment, intervention, education, treatment, consultation and referral for clients experiencing alcohol and drug or concurrent disorders or other behavioural health issues, to their families / caregivers and to other community Health & Human Service providers
