I am a HCPC registered paramedic currently based in the East of England Ambulance Service Hazardous Area Response Team.I am research active, having recently concluded my most recent project. This involved co-ordinating an award winning research trial aiming to bring ketone meters to frontline ambulances and improve pre-hospital hyperglycaemia care which has just been published in Diabetic Medicine. I have also recently published a literature review with evidence based recommendations into the implementation of BBE within pre-hospital care contexts and have a textbook chapter pending. Aside from my research activity, I have special interests in clinical leadership, change management, and pre-hospital mental healthcare.

I secured an 18 month secondment as a Research Paramedic to co-ordinate the KARMA2 trial: a feasibility study looking at 30-day re-admission and mortality of patients who have undergone pre-hospital point-of-care ketone testing by front line ambulance clinicians.This role involved planning the delivery of the study, data collection, maintaining databases, design and delivery of a training package for operational clinicians, act as liaison and point of contact with operational clinicians and hospital colleagues and undertaking qualitative research. I also collaborated with other members of the research team on studies including PARAMEDIC-3, STRETCHED, TRIM, & CESSATION.The main KARMA2 conclusions were:- Ambulance capillary blood ketone meter use for ‘High-Risk’ DKA patient recognition and commencement of fluid therapy was deemed acceptable.- Ambulance ketone measurement offered opportunities to expedite in-hospital emergency DKA care.- Ambulance clinicians associated meter use with improved clinical decision-making, patient support, and safety-netting.- There is scope for improved ambulance diabetes education – ‘sick day rules’ and euglycaemic DKA were unfamiliar for most study participants.- COVID-19 impacted study recruitment so that progression criteria were not met. Learning could include use of alternative trial design and clinician training mode for future research.This piece of work won the Diabetes UK Nursing and Allied Healthcare Professional Diabetes Research Award presented at the Diabetes UK Professional Conference!

Here I facilitate troubleshooting in any of these area's which can involve anything from signposting to information, mediation between 2 parties and even direct involvement on the behalf of a student. I have learnt mentoring and advising skills as well as knowledge and practise of confidentiality in the context of welfare maintenance. Here, I have also gained experience of effective case sheet writing and record keeping from this roles requirement of having to keep detailed notes for each appointment for future reference as cases often are more complex, requiring multiple appointments which may not be with the original advisor.
