Develop sound working relationships with clients, physicians, case managers and community members.Educate clients, physicians, case managers and community members regarding the benefits of home care services and appropriate patients to refer to home healthMake initial contact with other health care providers regarding service agreements and financial arrangements to ensure appropriate services are deemed necessary to meet the needs of the patientsPromote open communication between all departments to ensure proper implementation of new contractsWork with Director of Clinical Services to ensure optimal clinical outcomes for all clientsServe as a member of the Governing Body and participate in planning overall development of the office operations
