Leading a team of professional care managers in the delivery of home care services for recipients of Commonwealth Governement Funded Home Care Packages across the Melbourne Metropolitan and Gippsland Regions. Key responsibilities in this position was providing professional supervision, practice guidance, mentoring and coaching to a team of 16 Care Managers from diverse professional and cultural backgrounds. Ensuring our Care Management services were delivered in accordance with the Home Care Package guidelines and the Aged Care Standards.

My current role as Team Leader of At Home Care with VMCH is focussed on leading a team of professional case managers to deliver quality in-home support services to aged clients located across both the Melbourne Metropolitan and Gippsland regions. We provide in-home support services to clients funded by Commonwealth Government Home Care Packages and Commonwealth Home Support Program. Our primary goal is to assist people to live independently in their own homes by providing them with the support services that best meet their identified care needs.

Assignment as a locum case manager with Calvary Community Care based at their Morwell office to implement new Aged Care Home Packages on a CDC (Consumer Directed Care) basis across the catchment area.

Assignment working as a Case Manager with The Manningham Centre. The MCA provided Home Care Packages to eligible consumers to enable people to continue to live independently in their own homes.

Developed and delivered a new case management program to enable the ASRC to provide intensive support for asylum seekers living in the community who had exhausted their legal options to remain lawfully in Australia and were at risk of deportation and/or detention.

Manage a team of professional assessment officers for Council's Aged and Disability Services. Manage Council's Intake and Assessment Office and provide specialist assessment and casework support services for Council's HACC Program. Provide high level strategic advice to senior management responsible for the development and delivery of Darebin's Community Services. Develop and maintain effective partnerships and service agreements with other key service providers and community agencies in the sector.

I was employed as a Case Coordinator within Council's Specialist Children's Services. The program provided a range of Home & Community Care funded services to children and young people with a disability assessed as eligible for Council's Specialist HACC services. My primary task was to provide specialist assessment and coordination of services to eligible clients, and to supervise and support a team of Disability Care Workers assigned to my caseload.

Coordinate the delivery of the Asylum Seeker Assistance Scheme to eligible clients, and supervise and support a team of professional caseworkers across multiple sites. The ASAS program is an income support scheme funded by the Department of Immigration & Citizenship and delivered under contract by the Australian Red Cross. The program provided intensive case support services and an income support schemes to eligible asylum seekers living in the community and awaiting for their application for refugee status to be determined. I was responsible for the coordination and allocation of a large and complex caseload and the effective delivery of casework services to a diverse and extremely vulnerable client group.
