Peace Aging Care Experts, LLC™ (PeACE), is a private agency which utilizes the skills and knowledge of Trish Colucci, a Registered Nurse and Certified Case Manager, with over 20 years of experience helping clients work out the tangles life presents.Trish initially meets with an elderly or disabled client in the home environment to assess his/her status. She reviews all medical issues to assure that appropriate medical attention is being carried out, and then develops a Plan of Care which addresses the client’s individualized needs along with a specific plan of how best to meet them. A plan of care may include: coordination of home care, installation of medical equipment, referrals to physicians or elder law attorneys, attendance at key medical appointments, updates to family and continued contact to ensure that any changes in the client’s status are addressed on an ongoing basis.I work with Elder Law Attorneys, assisting them with guardianships, consultations and Plans of Care for their clients in need of care management services.
