Queer and Trans Youth United in Power Fellowship (QTY-UP)The San Francisco Human Rights Commission’s Queer and Trans Youth United in Power Fellowship is an educational and employment access program created to expose transitional aged youth with lived experience of homelessness to life skills, training, and the preparation they need to participate in the workforce and be leaders of tomorrow.

• Managed On-Demand Pilot Program in partnership with the Department of Public Health, Department of Homelessness, and Human Services Agency• Guided strategic proposal development towards sustainable solutions for pilot program aim to reach chronically homeless San Francisco residents • Developed partnerships with local homeless shelters to strengthen treatment and linkage plans for clients with severe co-occurring disorders • Monitored electronic benefit application submissions for client caseload with Medi-Cal, Cal Fresh, CalWORKS and CAAP with MyBenefits Calwin system• Outreached and coordinated client navigation services for chronically homeless residents of San Francisco• Facilitated monthly provider and client engagement meetings to problem solve current service inequities in San Francisco's homeless response system of care• Prepared, compiled and presented detailed reports and data regarding client applications to coordinated linkage programs
