Case Manager
CurrentAs Valiant Living's Case Manager, my role encompasses a wide array of responsibilities aimed at supporting our participants' journey to recovery. I specialize in coordinating care, ensuring participants find meaningful work, secure necessary documents, and connect with recovery communities. I facilitate communication between participants and their spouses, coordinate legal services, and arrange transportation for medical appointments. I also serve as a vital link between our resident psychiatrist, clinical team, financial office, admissions team, and housing team, ensuring all departments work in harmony to provide comprehensive support.In addition to care coordination, I lead various groups to enhance participants' recovery processes, including project assignments, action plans, and process groups, leveraging my certification as a CPFS. My role extends to overseeing the integration of technology with our PHP and IOP clients, allowing us to monitor their needs, whereabouts, and actions closely, ensuring personalized and effective support.My commitment is to foster a nurturing environment where participants can thrive, leveraging my skills in strategic coordination, communication, and holistic care. My work is driven by a deep passion for making a significant difference in the lives of those on their recovery path, by providing them with the tools and support they need to succeed.
