I worked with children and their families who are at risk of placement breakdown in their home and are at risk of coming into care. The focus of the role was to support young people and their families to ensure that families can provide a secure and safe environment in which the young person can continue to live and thrive.All referrals came from social workers requesting support via 'The Edge Of Care Panel'.I also completed Family Group Conferences (FGC) with families to help support families to commit to a plan that they agreed to amongst themselves as requested by social services.Main areas of responsibility:• To work in the community and with the child / young person and their family, in the family home. • Assist in ensuring a safe, secure and accepting environment for children and young people is maintained • Participate in the assessment of each young person’s needs and regular reviewing individual care plans/programmes.• Receive young people admitted to the residential home in a sensitive/caring manner.• Develop and maintain good relationships with parents/family of young people. • Ensure that the wishes and feelings of young people are taken into account in all matters relating to their daily care, appropriate to their age and understanding.• Encourage individuals to accept responsibility for their self-care, personal growth and development appropriate to their age. • Ensure that good health is promoted and that the health needs of young people are identified and services provided to meet these needs. • Actively support young people enabling them to make successful and positive choices about their futures. • Ensure the welfare of young people is promoted, that they are protected from abuse and that an appropriate response is made to any allegation or suspicion of abuse or neglect. • Encourage and motivate young people to develop interests and leisure activities which can be sustained in the family home.

Worked in a number of Children's Homes run by Manchester City Council including several years in an UASC Children's Home.Unaccompanied asylum-seeking children or UASC are children who have fled their country of origin without the care or protection of their parents.
