Hospital Social Worker for Wiltshire Council part of the Integrated Discharge Service, based at the Royal United Hospital (RUH).

Final year university placement within an adult social care team promoting independent living.RESPONSIBILITIES/MAIN DUTIES- Working with clients, their families/carers and other professionals to provide a service that will improve outcomes for the client and their carers with an aim to promote independent living as much as possible.- Conducting care act assessments, carers assessments, care planning, scheduled and unscheduled reviews using my knowledge of relevant legislations such as the Care Act 2014 and the Mental Capacity Act 2005 to inform my practice and professional decision-making.- Speaking with clients, families/carers and professionals at the point of referral to gather information.- Keeping accurate records of clients on the secure system, respecting confidentiality.

Second year university placement with Julian House, supported housing for adults with a diagnosis of Autism Spectrum Disorders.RESPONSBILITIES/MAIN DUTIES:- Working on a daily basis with client and staff.- Advocating with and for clients, ensuring to adopt a person-centre approach, encouraging client to be actively involve in any decision-making in relation to the support they receive.- Supporting clients to develop life skills and the confidence to become more independent, preparing for independent living.- Working alongside professionals and relevant external agencies.- Supporting clients within the service through constructing and delivering support plans, working inline with the organisations requirements and reviewed every three months, using agency policies to inform my practice.- Contribute to needs/risk assessments and support plans being delivered, recording and monitoring impact through reviews and use of outcome tools.- Giving advice, information and providing an enabling service so clients can access appropriate services.- Responsible for ensuring rent and service charge payments are made and recorded in respect of the named client, following the petty cash procedure and using online Microsoft excel to keep records.- Maintaining client records, ensuring they are accurate and updated regularly using Julian House client record system, always being aware of confidentiality.
