There are so many words to describe me, but awesome is my favorite! I am a United States Veteran, Mental Health Professional and Career Coach! Ten years of experience in customer service and patient-facing roles such as case management, discharge planning, referral relations, admissions, outreach, etc. Successfully directed and maintained a total of nine residential group homes in accordance with the company’s, Medicaid, the state and federal laws and policies. Eight years of experience in crisis prevention and intervention. Possessing the knowledge, skills and abilities needed to successfully thrive in a fast-paced environment and learn quickly results-oriented mindset, with experience working against concrete targets. Expert in working, facilitating, and training in both face-to-face and virtual environments. Strong abilities and passion to communicate professionally and collaboratively for the purpose of delivering a seamless, supportive experience and take on the challenge of the mental health crisis and play a formative role in providing live-saving solutions.

   Ensures assigned members have great experiences seeking and receiving care. Complies with escalation protocols and supports other navigators in identifying and handling high-intensity situations. Adheres to the processes, policies, and procedures of the program, including the protection of personal health information. Creates mental health content for the company’s newsletter each month. Documents all case interactions for evaluation and reporting purposes according to standard operating procedures in the Salesforce Health Edge and Virtual Benefits Administration. Engages in ongoing professional development to continually improve the ability to support individuals within a changing healthcare. Encourages members to engage more with their health by providing them tools and educational courses needed to embrace healthy lifestyle behaviors. Works directly with Curative physicians and other Curative clinical staff to administer personalized, preventative care. Follows up with patients after their health appointments to assist with any follow-up steps, questions, and concerns to ensure all their needs have been met. Possesses extensive knowledge in technology products, clinical programs, and policies and the ability to communicate with them effectively. Uses Electronic Health Records to fetch and complete patient records and other tasks. Manages patient communications and answer messages in a timely manner. Works adhering to US regulatory and Quality System requirements. Continuously created a positive experience and relationship with each member. Proactively engaged with members to manage their care. Conducts research on complex issues across multiple databases to resolve member issues. Interprets and communicates policies and procedures to other parties based on knowledge of the organization.
