-Assess needs and reasons why client is accessing shelter-Complete 9-Step Diversion script and VI-SPDAT assessments on new and returning clients - Develop care plans and provide practical assistance in collaboration with the client-Refer clients to case management after initial phase of admission ( after 30 days) or when requiring more support-Develop strong relationships with both current and new partner agencies -Broker linkages to relevant community supports for clients-Assist clients with housing resources and some light follow-up for diverted or housed clients-Work collaboratively with partnering agencies and colleagues-Engage in client advocacy-Provide frontline support and perform additional duties as required.
