• Conduct psycho-socials, service plans, health, vocational, and self-preservative assessments, as well as develop and implement person-centered treatment plans, and supportive services through evidence-based client treatment practices for up to 46 tenant caseloads. All documentation was entered into the AWARDS resource center. • Facilitating individual and group counseling by providing psycho-education in relapse prevention, health literacy, and navigating mental health and substance abuse systems. Assessing tenants’ substance abuse, health, mental health, and other care needs and coordinating services with community-based resources along with the tenant’s general support systems. Assist clients in improving and maintaining self-care, interpersonal, and vocational skills. Assist clients in achieving housing stability.• Manage monthly reports, which demonstrate the tenant's progress based on mutually agreed long and short-term goals and the documentation provided based on HASA audit requirements. • Prepare minutes for meetings, notices, special reports, correspondence, and packaging of resource material as required, and collect rent from tenants, for various program activities.Key Achievements:Overall, my key achievements include connecting residential clients to necessary community resources, promoting community engagement, facilitating access to specialized care, fostering self-sufficiency, and supporting residents' spiritual well-being.
