Major Accomplishments(1) Key player in preparing the Agency for the successful Community Health Accreditation Partner (CHAP) 3-year reaccreditation survey, July 2016(2) Lead role in the Agency shift to ICD-10 coding, October 2015(3) Lead role in implementation of the CMS Face-to-Face Encounter Documentation, January 2015(4) Lead role in the CMS Home Health Probe and Educate Medical Review, January 2016(5) Successfully defended the Agency in regards to CMS additional documentation requests (ADR) Responsibilities(1) Conducts OASIS data review to ensure consistency and completeness (2) Conducts utilization review to ensure compliance of services with the Plan of Care(3) Conducts review of patient charts for final billing(4) Conducts oversight on Medication Reconciliation by evaluating consistency of medications and dosages with clinical records from inpatient facilities and doctors’ offices(5) Researches and updates the Agency on new Home Health Care regulations (i.e., Medicare, State and Federal regulations)
