Provide case management, Interview and Assess client’s needs, process intakes for Healthcare and other Public Assistance Programs. Work in a specialized MAC unit working with clients who have Mental Illness, Chemical Dependency, or Adult Protection Issues. Coordinates needed services. Work interdisciplinary with internal departments as well as external agencies. Acted as a county representative at Regions Hospital where I interview clients for Public Assistance Programs. Currently I work offsite 2-3 days per week at Hastings Family Service in a Pilot as part of the commitment to an integrated, person-centered eases to access delivery system. ( EDO) . Gather information to determine eligibility for Public Assistance according to state and federal guidelines. Provide appropriate referrals to other community organizations. I work independently as well as a team/unit member.
