Social Work Specialist
CurrentCounty Adult Assistance Programs (CAAP) provides short-term financial support and social services to very low-income San Franciscans with no dependent children. CAAP is the primary safety net program that provides support and services to individuals who are not eligible for other public assistance programs. CAAP includes four sub-programs that address the specific needs of these clients: General Assistance (GA), Personal Assisted Employment Services (PAES), Cash Assistance Linked to Medi-Cal (CALM), and Supplemental Security Income Pending (SSIP).• Provide specialized case management services to eligible clients.• Conduct assessments to develop appropriate case plan; prepare summaries of clients� functional limitations, barriers, and/or needs; make referrals to community services.• Educate and help participants navigate through program expectations, policies and procedures, and service resources.• Partner or collaborate with other internal agency staff, community organizations, collaterals, and service representatives to provide clients with the services they need.• Participate in regular case consultations to discuss client assessments and case plan.• Compile and submit statistical and narrative reports.
