Supervise a Team Leader and 17 Support Workers. Manage the rostering in conjunction with the Workforce Planning Officer. Manage consumers on the Commonwealth Home Support Program (CHSP). This includes intake, conducting assessments, writing care plans, completing other required documentation, conducting annual reassessments and the matching and rostering of a regular Support Worker. Liaise with consumers and their families, monitoring and documenting changes. Support consumers to ensure their needs are met, requesting a reassessment through My Aged Care for unmet needs. Complete clinical incident reports. Submit end of month reporting for CHSP invoicing.
