The Healthy Family Teams offer a universal service to children aged 4 -19 years reviewing health at key stages and providing information, advice and support about health issues. The service aims to improve health outcomes for local children and young people. This is achieved by assessing health needs, delivering public health messages and offering targeted interventions including the National Child Measurement Programme, support for emotional health and wellbeing, reduction in teenage pregnancy rates, reduction in prevalence of sexually transmitted infections, substance misuse and smoking.I am tasked to work with children and young people attending schools in Nottinghamshire, this includes children and young people regardless of their education provision living in the County. I work in partnership with other agencies and professionals. These have a role to play in managing the communication between health and education so that children and young people can achieve positive outcomes and reach their full potential.

Case management, liaising with, and providing health protection advice and response across a range of partner organisations, professionals and the public.Providing support and participating in the team’s formal case review process, leading ongoing longer term management of issues relevant to a defined geographical area.Contributing to the development, training and supervision and mentorship of others within the team.
