-Grant-writing with a focus on community health initiatives -Collection/review of community morbidity/mortality information to identify community health needs, deficiencies in service, and assist in planning for correction/improvement-Developing, planning, and implementing activities and special projects with the NYCDOH&MH-Establishing and maintaining effective working relationships with community-based organizations -Development and implementation of hospital’s Cultural Medicine program-Development and implementation of a comprehensive employee wellness program-Development and coordination of cancer screening programs for uninsured members of the community -Coordination and implementation of community health education and screening programs-Facilitation of support groups offered to the community, including cancer survivors, smoking cessation, women with heart disease, bariatric support group, and more-Planning, implementation, and assessment of Six Sigma initiatives

-Establishing and maintaining effective working relationships with community-based organizations to ensure an integrated community health program-Advising Executive Leadership on community development-Representing the hospital in various community activities related to the promotion of health and prevention of disease-Identifying and developing marketing strategies based on hospital’s goals and market characteristics -Conducting outreach efforts at health fairs, street fairs, and other community events-Promoting positive community awareness regarding hospital services-Planning, implementation, and assessment of Six Sigma initiatives

-Providing counseling to citizens age 65 and over-Development and implementation of community outreach program targeting the elderly of Queens-Writing grants to expand existing program and to develop and implement new efforts with major focuses on community outreach -Conducting outreach efforts at health fairs, street fairs, and other community events-Conducting group public educational services on a variety of senior health-related topics

-Assisting in the planning and implementation of Quality Improvement educational workshops for healthcare providers in the field of Asthma Care and Preventative Services-Planning educational presentations/materials for parents regarding lead Poisoning and Injury Prevention Services-Providing administrative support for Program Director at EQuIP (Education and Quality Improvement Project) as part of the D.O.H. Childhood Asthma Initiative

-Providing information, education, and assistance in the Patient’s Bill of Rights and Family Responsibilities, and Advanced Directives-Improving patient outcomes by offering support and consultation via the Bio-Ethics Committee on ethical issues that arise in the health care setting -Encouraging the donation of organs and tissue for transplantation by providing information and education to patients and their families about the Organ Procurement Organization -Monitoring patient’s perception of quality of care and services received via continual patient satisfaction surveys-Organizing and maintaining a Language Bank and other interpretive services for patients with limited English proficiency-Advocating for patient’s rights on issues that cannot be resolved on the unit level

-Informing legislators, decision-makers, key media and other New Yorkers about PPNYC and its mission-Planning and implementing grassroots communications and community organizing campaigns-Data entry, fundraising, and Special Events planning
