Stable Housing Case Manager
CurrentConduct assessments to develop and update personalized service plans in accordance with grant guidelines.Facilitate meetings and activities and make appropriate referrals to service agencies.Coordinate services, support, and resources.Monitor the service environment to enhance clients' ability to achieve their goals, develop life skills, and maintain household stability.Create an environment that promotes empowerment, self-sufficiency, and resilience.Engage in comprehensive, client-centered social work activities, and maintain consistent case file documentation and management based on best practices and Council on Accreditation (COA) accreditation standards.Advocate for clients and families to receive support.Conduct community and home visits as necessary for client needs.Provide follow-up and monitor service delivery.Using computer-based applications, prepare correspondence, case notes, narrative and technical reports, notifications, and related documents. In addition, collect, track, and report case data in the system.
