Provided services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing within the community setting. Provides supportive counseling, outreach home visits, crisis intervention, and advocacy. As case manager, I provided linkage, monitoring, coordination, and procurement of rehabilitation counseling, residential and housing support, vocational readiness, and placement support.
