I am involved in the continuum of care from the acute hospital to the community for non-communicable diseases. In the hospital, I handle specialized diabetes patients with training as clinical diabetic educator, non-surgical weight management patients and bariatric surgery patients on their journey. Patient centred care has always been my keystone value, to help others raise awareness on the link between their eating habits and their health in order to facilitate development of life long healthy habits. I believe that there is no one size fit all approach in management of chronic conditions and hence has been exploring behavioural psychology tactics such as motivational interviewing, mindfulness, CBT and ACT. This expertise is being extended to the community with a hand in designing the landscape for chronic disease education and training of staff to do so.
